Form in DW Version 3

hi all,

i need form our website. form request information (name, address, etc) consumers. recieve information email.

we use dreamweaver version 3 (not cs3). although think can create form on dreamweaver, i'm not sure next. use fatcow our internet server, if help. i'm loss here.

can me?
is there tutorial online?

i check dw manual , couldn't understand.

what should after have form created in dw?
